2K and Hangar 13 have announced ‘Man of Honor,’ the first expansion for Mafia: The Old Country, during Summer Game Fest 2026.
This expansion continues the story of Enzo Favara, now set in the winter of 1905, after he has proven himself to the Torrisi crime family. He is now tasked with assisting Ennio Salieri, whom players of the original Mafia game would know as ‘Don Ennio Salieri.’

This adds two new story chapters, and you can either jump into it after you reach a certain point in the main campaign of Mafia: The Old Country or save it for after you finish the game.
Man of Honor also expands the game’s ‘Free Ride’ mode, with Salieri acting as your point of contact for a new set of new challenges.
